Legacy Health Insurance respects your privacy. This policy explains how information is handled when you visit legacyhealthinsurance.com or contact us through the website.
Information you choose to provide
Our contact and appointment forms request your name, email address, telephone number, state, message, and consent to be contacted. Please do not submit Medicare numbers, Social Security numbers, policy numbers, payment information, diagnoses, or medical records.
How contact inquiries are processed
Cloudflare Pages and a Cloudflare serverless function receive and validate the form submission. Amazon Simple Email Service (Amazon SES) delivers the inquiry to authorized Legacy Health Insurance recipients. We use the information to respond to your request, prevent abuse, troubleshoot delivery, and keep reasonable business records.
Website analytics
We use Google Analytics 4 to understand aggregate website use and measure actions such as successful form submissions, appointment-link clicks, telephone-link clicks, and use of the quote tool. We do not intentionally send form answers, names, email addresses, telephone numbers, Medicare numbers, or health information to Google Analytics.
Medicare quote tool
The Medicare quote page embeds a third-party SunFire application. Information entered into that application is handled under SunFire's and the applicable insurance organizations' notices and practices. Review the notices shown in the tool before entering personal information.
Telephone and video consultations
Scheduled consultations are conducted by telephone or through a HIPAA-compliant video-conferencing service. Information shared during a scheduled consultation may be handled differently from information submitted through the general website form. Follow the privacy and consent notices provided for that consultation, and do not use the general form to send medical records or sensitive health information.
Technical information and cookies
Cloudflare and analytics services may process IP addresses, device and browser information, pages visited, timestamps, security signals, and cookie or similar identifiers needed to deliver, protect, and measure the site.
Retention and secure disposal
We retain general website inquiries for up to 24 months after the last meaningful interaction, then delete or securely dispose of them unless they are needed longer for an ongoing customer relationship, a legal or regulatory obligation, fraud or security prevention, a dispute, or another documented business need. Technical and security logs may be retained for up to 12 months or for the shorter or longer period required by the service provider's security schedule. Aggregated or de-identified analytics may be retained longer because it is not intended to identify an individual.
We do not sell personal information submitted through our forms. We disclose information to providers that help operate and secure the website or deliver email, when you direct us to do so, or when disclosure is permitted or required by law. When records reach the end of their retention period, we use deletion or disposal methods appropriate to the record and service involved.
Your choices and privacy requests
To ask about, correct, or request deletion of information you submitted, use the contact form and begin your message with “Privacy Request.” Describe the information and the action you want us to take, but do not send a Medicare number, Social Security number, medical record, or copy of an identity document through the form.
We may ask for limited additional information to verify that the request concerns you. We aim to respond within 45 days. We may retain information when reasonably necessary or permitted by law, including for compliance, security, fraud prevention, active service, or disputes, and we will explain any applicable limitation. You can also limit cookies through your browser settings; analytics controls may depend on your browser and Google settings.
